Oracle DB Error ORA-06578 output parameter cannot be a duplicate bind

The following is the cause of this error message:

The bind variable corresponding to an IN/OUT or OUT parameter for a function or a procedure or a function return value in a CALL statement cannot be a duplicate bind variable.

Action you can take to resolve this issue: Change the bind variable to be unique.
